- The environmental medium or media into which the substance was released or discharged.
- Potential health effects associated with the release or discharge of the substance.
- Precautions taken, including evacuation, remediation, or other proposed response actions (OEPA, 2009).
- Any additional information that facility managers can provide ICs, including but not limited to, whether the substances:
- are stored at ambient temperature and pressure and if not the specific conditions
- are solid, liquid or gas at ambient temperature
- specific gravity
- vapor density
- possible reactions between chemicals if multiple chemicals are involved
- any other information that may be helpful in controlling the release
